Introduction

These Terms and Conditions outline the agreement between Dignity Behavioral Health Specialist, LLC (“the Provider”) and individuals (“the Client”) regarding the provision of professional mental health care services. By engaging our services, Clients acknowledge and accept these Terms and Conditions.


Services Provided

We provide a range of mental health services, including but not limited to psychiatric evaluations and medication management. The specific services to be offered will be determined and agreed upon with the Client before treatment begins.


Confidentiality

All information shared during sessions is treated as confidential. Information will only be disclosed with the Client’s consent, except as required by law—for example, in cases involving suspected abuse, imminent risk of harm to self or others, or when disclosure is mandated by a court order.


Appointments

Appointments must be scheduled in advance and are reserved exclusively for the Client. Clients are expected to arrive on time. Sessions will end at the scheduled time even if the Client arrives late.


Cancellation Policy

If a Client needs to cancel or reschedule an appointment, at least 24 hours’ notice must be provided. Cancellations made with less than 24 hours’ notice may result in a $100 fee.


Fees and Payment

Service fees will be discussed and agreed upon before the first session. Payment is due at the time of service unless another arrangement is made in writing.
We accept the following payment methods: Zelle, credit cards, Cash App, and PayPal.


Insurance

We may accept certain insurance plans. It is the Client’s responsibility to confirm coverage in advance. The Client is financially responsible for any fees not covered by their insurance provider.


Emergencies

Our practice does not provide emergency services. In the event of a mental health emergency, Clients should call 911 or proceed to the nearest emergency room immediately.


Communication

Clients may contact us during regular business hours for non-urgent matters. Acceptable methods of communication include phone calls and text messages. We aim to respond to all non-urgent communications within 24 hours.


Termination of Services

Either the Client or the Provider may terminate services at any time. The Client is responsible for payment of all services rendered prior to termination. The Provider may also discontinue services if the Client does not comply with these Terms and Conditions or for other professional reasons.


Client Responsibilities

Clients are expected to actively participate in their treatment and to provide accurate and complete information necessary for effective care.


Privacy Policy

We comply with all applicable privacy and confidentiality laws governing the handling of personal and health information. Clients may review our full Privacy Policy for more details.


Dispute Resolution

Any disputes arising under these Terms and Conditions will first be addressed through mediation before either party seeks resolution through litigation.


Changes to Terms and Conditions

We may update these Terms and Conditions as necessary. Clients will be notified of significant changes, and the updated version will be posted on our website.


Governing Law

These Terms and Conditions are governed by the laws of the State of Nevada.

By engaging the services of Dignity Behavioral Health Specialist, LLC, Clients acknowledge that they have read, understood, and agree to these Terms and Conditions.
